Custom Reports Attached To Permit Module do not show up in Actions (Doc ID 1449571.1)

Symptoms

In the Permit module, 'action' tool bar, the associated Custom Report text is not displayed as it has been set in Modules Administration -> Forms -> Module Reports screen.
