BLANK SECTIONS In Expedited Report Forms

The expedited reports (CIOMS and MedWatch) generated by the Transaction server are not correct.
Some fields that were entered in the application are not printed on the forms when using the Report/Bulk Reporting function from the application menu bar.
The Draft reports scheduled in the Web server include all the information, no fields are missing.
However, when the same scheduled reports are printed and marked as submitted with the Bulk Reporting function some fields are not populated anymore.
The fields missing from the reports are always the same for all cases: patient's information, suspect drug information, case ID, date of report, date of event, reporter's information.
We switched the Print Service from 0 to 1, but the problem still occur.
