Symptoms
Individual Self Service Tasks are stamped with the Self Service User's email address at the time the Self Service Task is created. If the Self Service User changes their email address in their Profile, active Notification Self Service Tasks are not updated to reflect the current email address. This leads to notifications being sent to the wrong email address.
