Symptoms

 Customer reported missing dynamic forms throughout the study for various subjects. This was regarding a particular repeating dynamic form.

Changes

 There was a Post Production Change (PPC) which took place in the study. The specific global condition responsible for triggering the specific form was updated such that the triggering condition was on a different form then it was originally attached to.
