User Fails to Login With Error "The Session Has Been Terminated. Please Login Again" With Fatal NI Connect Error 12170 in DB Alert Log (Doc ID 2467143.1)

Symptoms

Agile Users could not login to the PLM login page.
The application login page was up and running, however when user tries to login, gets below error.
The problem happens once in a while. After restarting Agile Application Server and database server, the system is running fine.

Error

The session has been terminated. Please login again

Summary of related error messages found in application server logs around the time the problem happened:
javax.transaction.xa.XAException: The connection has been administratively diabled. Try again later. The action was taken by a console or administrative command. java.lang.Exception: It was disabled at Tue Oct 02 03:03:06 GMT 2018
javax.transaction.xa.XAException: Two rollbacks failed. The first one threw Closed Connection. The second one threw Closed Connection

<Test "ISVALID" set up for pool "opss-audit-DBDS" failed with exception: "isValid returns false".>

1. Launch browser, access to Web Client and login
