Need OSM OTM Doccumentation (Doc ID 2562492.1)

One needs more details or documentation on Order Transformation Manager(OTM) which can answer what happens in case of below errors related to OTM.

In case of missing fields(mandatory), will service order be generated or not.
If OTM fails, what does it mean to the whole order? Would it wait for revision? Is Siebel supposed to revise? Would order move to fallout state?
