Dataconnect Consumption Extracts Not Working (Doc ID 2710392.1)

Goal

When attempting to add an adhoc consumption extract, after selecting request type, user is taken to the screen to add a new request _type_.  Would instead expect to navigate to the screen for adding a new consumption extract request for the given request type (i.e., to select consumption extract type and start/end date/time.
