Allocation Screens Blank After Applying 8.0.7.4.2 or 8.1.0.1.0 AAI and PFT Patches

Symptoms
On PFT 8.0.7.4.2 (80742) or 8.1.0.1.0 (81010) after applying several patches, many of the allocation screens are coming up blank.
The only screens which do not work are below. Other screens are coming up fine.
Rule Specification ===> Allocation Specification ===> Allocation Model ===> Static Table Driver ===> Lookup Table Driver

The issue can be reproduced at will with the following steps:
• AAI = Upgrade from 8.0.7.2.0 ======> 8.0.7.4.0
• PFT = Upgrade from 8.0.7.2.7 ======> 8.0.7.4.0 ======> PFT 8.0.7.4.2 One-Off Patch (Static Table Driver issue)
