P6 EPPM Database Upgrade to 20.12.x on SQL Server Fails With Error "The index 'ndx_doc_2' is dependent on column 'doc_uuid'" (Doc ID 2814486.1)

Symptoms

When attempting to upgrade a Microsoft SQL Server database from 19.12.x to 20.12.x, the following error occurs (Note: the exact script name depends on what P6 version+patchset the database is being upgraded from).
