Patient Data Report (PDR) / Blank Casebook Report (BCR) shows no data or truncated data for Header fields (which have visual appearance of 'Box Set' or 'Comb' in FLT). Following example is from the PDR:

Truncated Header data which appears on a multi page layout increments in line with the page number i.e. on page 1 the first character of 'Patient' and 'Site' appears while on page 2 the second character of 'Patient' and 'Site' appears etc (compare above PDR screen capture to the below one):
