Navigating Within OPPM, Getting Redirected To Organization URL Ending In "phishing.asp" (Doc ID 2905055.1)

Cannot create or edit categories.
Cannot change Admin Password or Add users to OPPM.
Getting Error 404 web page when trying to navigate.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:

Attempt to create or edit categories.

OR

Within Setup>Users
Click on Administrator, System, then click on setup

Double click the Administrator, System - login admin account

Clicking on New, Edit, or Duplicate produce the same 404 page not found error.
