Stitched PDF Is Not Displaying In PDF Output

Symptoms
When generating PDF output that include attachments that are PDF stitched (native PDFs; not using AddMultiPageBitmap).
We have a custom UI that our ad-hoc drafters use to generate documents. We allow them to add PDF attachments to the document that they are drafting. We are now using PDF Stitching to attach the document.
When the Drafter 'Proofs' the document we can see it attached to the document. But when the document is submitted and the TRNS goes to 999 status, the PDF that is produced does not have the attachment; it has a single blank page.
