Upgrade Fails for External Presentation Server with Mediawiki Database Error (Doc ID 2979856.1)

Symptoms

When upgrading an existing External Presentation server  the installation may fail to complete with a error message similar to the following that can be found in your $A1BASEDIR/logs/Package.log.

Note: $A1BASEDIR refers to the folder where the Unified Assurance software has been installed.
