FRM-92100 WHEN SAVING DATA IN DATA ENTRY; FATAL ERROR RE-DEFAULTING LAYOUT

Symptoms
You receive the FRM-92101 message after entering the last field on the DCM and tabbing out of the field.
When you click OK on the message you get kicked out of OC. You have to log back in again.
Not able to save data in data entry on particular DCM(s)
- When saving data they receive the following error message:
FRM-92100: Your connection to the Server was interrupted.
A warning message
'FATAL ERROR: Can't generate DCM with no questions' appears when tried to generate the layout for the DCM
