PDR Fails With Errors MSG-00005 & REP-1419 If DCI Long Name Has Special Chars (Doc ID 747408.1)

Symptoms

On 4.5.3 in Production:
When running a Patient Data Report (PDR), it is failing with the following error in the log file:

STEPS TO REPRODUCE

  1. Create a DCI with the name which includes an ampersand symbol (&) e.g. "RIGHT HEART CATH & VITALS"
  2. Generate the DCI Provisional Layout and Form
  3. Activate the DCI
  4. Create a DCI Book and add the DCI created in step 1
  5. Activate the DCI Book
  6. Create patients and enter data via RDC HTML
  7. Run the Patient Data Report for the patient => Error
