 What can cause an "Already Logged In" error?
 What causes an account to not log out properly (ie. CES_USER_LOG) and the session to remain open?
 Sometimes open sessions in CES_USER_LOG are overriden when a user logs in.  Other times they get a message they are already logged in.  What can cause this?
