Invalid entries in the Sybase interfaces file -- UTILITY server, Open Server Error 16013 (Doc ID 949445.1)

Symptoms

This is a fatal error that occurs when trying to start ASAP (using the script start_asap_sys (-d)). After issuing the command, nothing occurs (the start ASAP command seems to 'hang' and nothing (including no error message) is returned).

$ start_asap_sys -d
Starting Master Control Server CTRLDV01...

In this situation, no individual ASAP server can start up since there is an error with the startup of the UTILITY server.
… and the following will be the only entries in the $ASAP_BASE/DATA/logs/ASAP.Console file …

$ cat $ASAP_BASE/DATA/logs/ASAP.Console
CTRLDV01 Server: Fatal Server Error:
Open Server Error # [16013] Severity [20] State [0] Error Text [Could not find server name 'CTRLDV01' in interfaces file
]
CTRLDV01: srv_run() Failed
