JOB STATUS FAILURE FOR HTML FORM GENERATION UPGRADE UTILITY (Doc ID 949613.1)

-- Problem Statement:
Trying to generate PDR templates using using the HTML Form Generation Utility (OCDCITEMPLATE.HTM). Some PDR HTML are succesfully generated, but now the process has stopped, failed or has hung.


-- Steps To Reproduce:
1. Login as any super user account 
2. Query and select problem Study
3. Click on the [Generate] button 
4. Check the job status and log file 
5. Job status = "FAILURE" or "STARTED"

The log shows the following:

Env:
Report Server being used: <Report Server>
Datebase: <Database>
Username: OPS<UserName>
Image Folder path: \\ocprod\rdc\dcif_images
Image URL path: http://ocprod/opa45/rdc/dcif_images
Html Image URL path: crfimages
