Approve/Reject Buttons In Requisition Approval Notification Email Are Not Working Correctly

Symptoms

On : 11.1.11.1.0 version, Approve Requisition

ACTUAL BEHAVIOR
---------------
The email notification body contains 2 sets of approve/reject controls.

unable to approve or reject Requisition from email notification using the approve or reject icons.
the email generated has To = reject_mailtoURL


EXPECTED BEHAVIOR
-----------------------
be able to approve requisition from email

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. submit requisition for approval
2. from email click on reject button(icon)
3. the email that is created doesn't have TO email address instead has reject_mailtoURL

BUSINESS IMPACT
-----------------------
The issue has the following business impact:
Due to this issue, users cannot approve or reject requisitions using email

Changes

 Requisition Approval Notification Email layout is changed in Release 11 and above
