Attaching A New Storage Volume To An OCI Compute Instance Fails With Error "Conflict - Volume [ocid] currently attached" (Doc ID 2388362.1)

Symptoms

Attempting to attach a new block volume to a running instance.  We were able to create a new block volume but when we try to attach it to an instance we get an error:

"Conflict - Volume [ocid] currently attached"

Checking the storage volume shows that it is not attached to any instances.
