Can't Assign IDCS User To Fusion Application, error "Account Creation Failed" (Doc ID 2409629.1)

Symptoms

On : NA version, Group Management

When attempting to synchronize users between Fusion and IDCS, the synchronization is not working. The following error occurs;

ERROR
-----------------------
 "Account creation failed".


STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1) Integrate FA application with IDCS
2) Enable synchronization
3) Create user in IDCS (same as user previously created in FA)
4) Execute a synchronization
5) Review results of the synchronization, note user not assigned.
6) Manually attempt to assign the application to the user.
7) error returned to the UI: "Account creation failed".

BUSINESS IMPACT
-----------------------
The issue has the following business impact:
Not able to provide SSO access to Fusion Apps users.

Changes

 Need to delete and recreate both Fusion and IDCS users.
