Symptoms

Performance Goals R13: Manager unable to approver goal submission by worker.

There is not change to the approval rule configuration. Worker submitted a new goal for manager approval and we are seeing following issues

1) No email notification is triggered to manager
2) No entry in BPM as a result no 'Bell' notification
3) Manager does not have an approve option from My Team page as well
