Serial Uniqueness Got Violated For Item

Why is it that the same serial numbers that were used in the first transactions can be seen and used again?
Customer performed miscellaneous receipt transaction earlier on an item and associated some serial numbers with it. Later, performed Direct organization transfer on entire quantity.
Post this, again performed miscellaneous receipt transaction to see what is posted in the question above.
Customer had setup Serial Uniqueness to be Within Inventory Models and Items, and so the system should not have allowed to Receive with the same serial numbers.
