Symptoms

A Transition service's results fields are not showing when attempting to add fields to a form.

Changes

 A transitions service was already activated in the zone (and being used).  Subsequently another transitions service was activated and is the one with fields seemingly not exposed for being added to forms (e.g., Field Chooser) or available for transitions process steps/task configuration.
