Resource Repeated on Account Team

We are seeing an issue where team member is repeated many times on a given account. This appears to only be for accounts that have been created via conversion to master data and are then accessible in Oracle Sales Cloud.

We need to understand why team member has been repeated so many times and prevent this from occurring in the future. We only see this on accounts when accessing through a dynamic choice list that we created to tag additional accounts to an opportunity for reporting purposes.
