FYI Notification not received for Termination Workflow

Information in this document applies to any platform.
The customer's business requirement is to send an email to a group of employees with a specific role when employees are terminated.
They created a new user account and role for this purpose.
They also added another step to each of their termination workflows which is an FYI notification to all users who have the new custom Application Role.
They tested this by submitting a termination, which was approved and completed.
When reviewing the completed transaction in Transaction Console, found that the FYI notification step failed.
