The issue can be reproduced at will with the following steps:
1. Setup: Manufacturing and Supply Chain Materials Management

2. Functional Areas: Inventory Management > Manage Item Transaction Defaults
  Actions > Import from CSV FIle

3. CSV Upload
4. Submit
