OAC-Classic: Deletion Of Instance Failed With Error "PSM-LCM-02101"

Symptoms
OAC-Classic (Cloud At Customer) / OAC-Classic
Delete service operation of the instance failed with below error :
Unable to stop the following Compute resources: /Compute-<INTERNAL_ID>/<USERNAME>/paas/ANALYTICS/<INSTANCE_NAME>/BI/user-tresources-1
PSM-LCM-02101 Unable to delete service [[post-delete]]. Check additonal reported errors.

But the deleted instance is not showing up in the list of instances available. When trying to create a new instance with the same name, it failed with below error :
The Service <INSTANCE_NAME> has failed resources from previous creation. Please choose a different name for the Service OR complete the cleanup to reuse the same name.
