Symptoms

When attempting to generate a token, the following error occurs.

Error sending message to endpoint https://SALESFORCEURL.lightning.force.com/services/oauth2/token.

This occurs after the initial setup for managed package 8.x.

Changes

This is either a new setup or the package was upgraded to version 8.x or later.
