Errors encountered when a Submit button is clicked in an Intelligent Advisor interview

ERROR

An error occurred while trying to process the request.
To get more help, enter into contact with the assistance service.

In Intelligence Advisor log, the following error shows: OPA-DATA-108
Error for policy model <DEPLOYMENT_NAME>:<X> - Error parsing response from data service Deployment: <DEPLOYMENT_NAME>:<X>

The interview should run and when submitted, it should create a Lead in CX Sales.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Run the interview
2. Answer the questions
3. Click in Submit
Error occurs
