PRC:POZ: Vendor Unable to Create Work Confirmation -error: Failed to Get Requested Information (Doc ID 2772655.1)

Symptoms

Vendor unable to create work confirmation as line item disappeared (no items to display) and all the blank box (ie: Work confirmation Date, Work Completion Date, Supplier Reference, Comment, Attachment) can't be filled in.
Once click new work confirmation (WC) button, a new WC number has appeared (OCM20000080_15), but vendor cannot fill in any information and upload any attachment (all become grey), hence the WC can’t be saved and submitted

Error popped out:

” Failed to get requested information”

Purchase Order line item not appeared for all previously approved Work Confirmation.
