Oracle Fusion Learning Cloud: Approver Receiving FND-2 Error On Learning Request Approval (Doc ID 2795520.1)

Symptoms

A Learning Specialist will receive a learning request and select "Claim" to take over the approval. After reviewing the request, the Learning Specialist sends the request back to the Learner to request for additional information.
The Learner completes the request and re-submits it but when the Specialist tries to submit it for approval or request for more details, they get the following error:

An error occurred. Contact your helpdesk. (FND-2)
