Symptoms

Approved Requisition remains in Pending Approval status even though it was approved.
Approver approves Requisition by email, and receives another email with an error.

ERROR

"The message you sent appears to be un-solicited. If you are responding to a notification, use the response link that was included with your notification.
The response message is not from the original account to which notification had been sent. Actionable notifications should be responded from the same account to which it had been sent."

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Create Requisition
2. Approval email is sent to Manager
3. Manager clicks on Approve button in the email
4. Auto reply:
The message you sent appears to be un-solicited. If you are responding to a notification, use the response link that was included with your notification.
The response message is not from the original account to which notification had been sent. Actionable notifications should be responded from the same account to which it had been sent.
5. In the UI, the Requisition shows status of Pending Approval
