ACTUAL BEHAVIOR

2 ref item has been created as primary ref item.
EXPECTED BEHAVIOR
-----------------------
There should be one primary ref item.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Create item through API.
2. Push 2 ref item in separated in very short interval of time.
3. Noticed 2 item created with primary ref indicator as Y.
