PROBLEM STATEMENT

Duplicate forfeiture records after withdrawing and recalculating accruals in the last plan term accrual period.




 
STEPS
-------
The issue can be reproduced at will with the following steps:

1) Withdraw the accruals in the last accrual period of the plan term
2) Recalculate the accruals
3) A new forfeiture row is added with each withdraw and accrual run
