Check Box Field Doesn't Retain the Check on the Final PDF Form (Doc ID 2935080.1)

Symptoms

When employee fills out a configurable form that has checkbox fields, the checks are not retained when the final pdf form is created. When a form is signed the checkboxes on the final pdf appear blank.
