Symptoms

Duplicate relationship (e.g Account, Contact, Opportunities, Lead) are added on a custom object after 23A

You have a custom object with some subtab based on many to many relationships
After 23A when you add a child record on this custom object, if you click on Apply then ok button the record will be added 2 times.

Changes

 After 23A upgrade
