PRC:SQM: Initiative Owner Displays Multiple times with Wrong Job Titles (Doc ID 2943200.1)

Symptoms

When creating an Initiative, the Initiative Owner Job field in the Evaluation Team tab does not match with the user's Active Job on their User Account Setup.

For example: when creating an Initiative, User's job title shows: Accounts Payable Rep., but on their User Account Setup, it is: Vendor Management Program Manager (their current job).

STEPS TO REPRODUCE
----------------------------
1. User creates a new Initiative
2. Job shows: Accounts Payable Rep
3. User Account Setup shows their Active Job Title as Vendor Management Program Manager (correct)

Note: If the team member is deleted from the Initiative and then added back, they will then display on the Team tab multiple times, all with different job titles.
