New Store Unable To Add Address

Symptoms
ACTUAL BEHAVIOR
---------------
Customer is unable to add the Business address for New store. Looks like the Add button in address window is disabled.
EXPECTED BEHAVIOR
-----------------------
Customer should be able to add address.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Go to Location Foundation
2. Select > Organization Hierarchy
3. Navigate to Store -> Create -> Address
