Workflow Went to Employee after Manager's Approval

Customer is reporting below issue from approval history of Resignation transaction and now wants to know the RCA.

In the transaction console’s approval history, the resignation flow shows that it went back to the employee after manager's approval. Why does the approval of resignation go back to the employee again? It should be Employee > Line Manager and self auto approve and done.


EXPECTED BEHAVIOR
-----------------------
Approval should route like Employee > Manager > Self auto approve

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Resignation > Approval flow should be as Employee > Line Manager > self auto approval
Approval History of the transaction shows Employee > Line Manager > Employee.
