ACTUAL BEHAVIOR

Field is not appearing under the Personal Details region for the 'Add Pending Worker' quick action even though it is set to be visible through Design Studio.

EXPECTED BEHAVIOR
-----------------------
Field should show as it is enabled in Design Studio.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Enable field in Design Studio for the Add Pending Worker quick action
2. Field does not show correctly as per setup.
