Symptoms

On OIC Gen2, users can approve/reject tasks via
 - Click the button within the email notification, no login required.
  or
 - Reply to email

On OIC3/OPA, users can approve/reject tasks by clicking a button within the email notification, but the user must log in first. There is no reply to email option.
