FAW upgrade, sometimes, may have been failed/in-progress. sometimes, the Upgrade Content <Version Number> in the SME activity may have been failed.

This can be seen using the below navigation:

Login to FAW/NSAW Application and navigate to Console --> Activity --> Upgrade Content <Version Number> --> "State Failed".
