Some Assets Are Created Without Content/Media Type Being Set (Doc ID 2994041.1)

Symptoms

The customer has reported that some documents being uploaded through the create a digital item REST API (/content/management/api/v1.1/items/) as assets that don't have the media type being set.

This causes a few issues:

1) The document thumbnail to be displayed as an "X"
2) The user can't preview the file in the browser
3) When downloading the asset the file is downloaded without the extension and with the name set as the Asset ID
