Fields Are Missing From The Manage Knowledge Content Types Page (Doc ID 3000338.1)

When attempting to create a new Content type in the 'Manage Knowledge Content Types' task fields are missing from the page.
Users are getting an error when trying to save the content type due to missing fields in the setup page.

OK-GEN0004 : must not be null
OK-GEN0016 : Cannot perform operation because of invalid input.

The issue can be reproduced at will with the following steps:
1. Navigate to setup and Maintenance 'Manage knowledge Content types' task
2. Create a content type.
3.  Save. error occurs
