Contacts Are Missing The "Job Title" and "Phone Number" on My Open HR Help Desk Requests

Symptoms
When adding a contact on My Open HR Help Desk Requests> Pages > Contact. For some reason the Job Title and Phone Number are missing as shown in the screenshot below:
Step to Reproduce the issue:
1. Go to Help Desk > Help Desk Request.
2. On My Open HR Help Desk Requests > Drilldown into Service Request.
3. Go to Pages.
4. On Contacts > Click " Add Contact" button.
5. After adding a contact for some reason the Job Title and Phone Number are missing or empty.
