ACTUAL BEHAVIOR
---------------
When going to a Business Process (BP) and navigate to a line item, click on add and go to attachments tab, there you can see a file attached by default.
EXPECTED BEHAVIOR

When going to a BP and navigate to a line item, click on add and go to attachments tab, there should not be any file attached by default.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
- Log into the environment
- Go to a BP and navigate to a line item
- Click on add
- Go to attachments tab;
- You can see a file attached by default.
