Symptoms


ACTUAL BEHAVIOR
---------------
HR representatives do not receive notifications when an employee is reporting an incident.

EXPECTED BEHAVIOR
-----------------------
HR should be able to get alerts when employees are reporting incident.


STEPS
---------
The issue can be reproduced at will with the following steps:
1. Log in s employee: Go to Me > Safety Incidents> Create an incident
2. Go to My Client Groups> Safety Incidents>Test Incident >Events > notify HR
3. The alert is not getting executed
