Symptoms
AQ PL/SQL Notification stopped working after a shutdown and restart of the database.
select * from DBA_SCHEDULER_JOB_RUN_DETAILS where job_name like 'AQ$_PLSQL_NTFN%';
reveals that a notification job was stopped at the same time as the instance was shutdown with the following reason
AQ$_PLSQL_NTFN6677 STOPPED REASON="Job slave process was terminated"
