CRS Fails to Start With Error PROC-22 or PROC-26 (Doc ID 1367262.1)

CRS fails to start on any node after storage maintenance:

1. In /tmp/crsctl.NNN:

Oracle Cluster Registry initialization failed with invalid format: PROC-22: The OCR backend has an invalid format

2. In $CRS_HOME/log/<nodename>/client/cssNNN.log:

2011-10-10 22:38:45.975: [ OCRRAW][1]propriogid:1: INVALID FORMAT
2011-10-10 22:38:45.976: [ OCRRAW][1]proprioini: disk 1 (/dev/rlv_ocr2) does not have enough votes (1,2)
2011-10-10 22:38:45.977: [ OCRRAW][1]proprinit: Could not open raw device
2011-10-10 22:38:45.977: [ default][1]a_init:7!: Backend init unsuccessful : [26]
2011-10-10 22:38:45.977: [ CSSCLNT][1]clsssinit: Unable to access OCR device in OCR init.PROC-26: Error while accessing the physical storage

3. ocrcheck reports below error:

$ ocrcheck
PROT-602: Failed to retrieve data from the cluster registry

4. The permission and ownership of the OCR disks are correct on all nodes:

# ls -l /dev/rlv_ocr*
crw-r----- 1 root oinstall 46, 15 Oct 10 18:42 /dev/rlv_ocr1
crw-r----- 1 root oinstall 46, 16 Oct 10 18:32 /dev/rlv_ocr2

5. The /etc/oracle/ocr.loc has correct entry on all nodes.
